Seeking one part-time care managers to be part of a community-based team to provide intensive case management services to individuals with schizophrenia and chronic medical conditions who have recently been hospitalized for a medical illness and are enrolled in a research study.

 

Duties/Responsibilities:

Meet with patients in the hospital prior to discharge to home to facilitate and support the discharge process
Communicate with the hospital inpatient staff to coordinate discharge planning and aftercare follow-up
Complete needs assessment to identify barriers and obstacles to successful transition to community care
Meet with patients in the community upon discharge and routinely for 3 months to support and problem solve around connections with aftercare medical and mental health providers
Communicate as indicated with medical and mental health providers to coordinate appointments and facilitate patient adherence with treatment recommendations
Document all service planning, collaborative goal setting, linkage, advocacy and mediation activities carried out with and on behalf of the client
Participate in training and supervision activities with other case manager and supervisors

 

Minimum Qualifications:

Bachelor's degree
2 years experience working with individuals with severe mental illness in inpatient, outpatient, clinic, or case management, or other support capacity
--has a firm understanding of the personal challenges and systemic barriers associated with obtaining help for behavioral and other health conditions as well as other areas of need including income, housing and social support
is knowledgeable about community-based resources for mental health and primary care in the Bronx
is adept at interdisciplinary team work
